Insurance Information

If you are Contracted with Blue Cross & Blue Shield Texas,

Ambetter/Superior Health, United Healthcare, Tricare or Medicare...

If you are contracted with any of the above insurance carriers, call the number on the back of your card and ask the following questions to verify coverage before booking your first appointment.

Does my plan cover outpatient medical nutrition therapy?

If yes, how many sessions are allowed?

Does my plan only cover visits that are considered “medically necessary?"

Do I have a deductible to meet first? If yes, how much?

Do I need a physician referral?

What is my co-pay amount for outpatient nutrition counseling?

If you are not Contracted with the Above Insurances

If you are not contracted with the above insurances and you would like to inquire about out of network reimbursement, call the number on the back of your card and ask the following questions to verify reimbursement before booking your first appointment.

Optional Script: Hello, I am interested in seeing an out-of-network Registered Dietitian Nutritionist.

Does my plan provide reimbursement for out-of-network providers?

If yes, make sure to ask the following questions:

How many sessions does my plan cover, and how long are the sessions?

What is my deductible, and has it been met?

What is the coverage amount per therapy session?

Do I need pre-authorization from my insurance company or PCP?

For your own peace of mind (and organization) make sure to ask your insurance company exactly how to submit your Superbill (the detailed receipt that I will provide you after your sessions) for reimbursement.

Optional Script: After my appointment with my Registered Dietitian Nutritionist, where do I send the Superbill for reimbursement, and when can I expect to be reimbursed?

Pay Policy

If you are planning on receiving out-of-network reimbursement for medical nutrition therapy, or you plan on self-paying for your session, you are responsible for payment at the time of your service. I accept cash, check, or credit/debit card.
